RISE PROGRAM
The 1915(i) RISE Initiative is a Kentucky Medicaid program designed to support adults living with serious mental illness (SMI) by promoting recovery, independence, and meaningful community engagement. Available to individuals ages 18 and older who are enrolled in Kentucky Medicaid and have a qualifying diagnosis such as schizophrenia, bipolar disorder, or major depression, the RISE program offers personalized, comprehensive support tailored to each individual’s needs. Services may include case management for goal setting and coordination of care, assistance with finding and maintaining housing, in-home support for daily living, employment or educational guidance, medication management, and access to community resources like transportation and peer support. Through these services, the RISE Initiative empowers individuals to live more independently, build stability, and reduce reliance on institutional care while improving overall quality of life.
