1915(i) RISE Initiative Services in Kentucky
The 1915(i) RISE Initiative is a Kentucky Medicaid program designed to support adults living with serious mental illness (SMI) by providing recovery-focused, person-centered services that promote independence, stability, and community engagement. Through individualized care planning and ongoing support, providers help participants build the skills and resources needed to live successfully in their own homes and communities—reducing reliance on institutional care.

Whether individuals need help maintaining housing, managing daily routines, or working toward employment or education goals, RISE services are tailored to meet them where they are and support long-term success.

How RISE Services Support Participants:
Case Management & Care Coordination

Personalized goal setting and service planning

Coordination of medical, behavioral health, and social services

Ongoing support to ensure progress and accountability

Housing & Tenancy Supports’
Assistance locating safe and stable housing

Support with maintaining leases and preventing eviction

Guidance navigating housing resources and applications

In-Home Independent Living Supports
Help with daily routines such as hygiene, meal planning, and household tasks

Skill-building to increase independence and confidence

Support creating structure and stability at home

Supported Employment & Education
Job readiness training, coaching, and placement support

Assistance with GED programs, college enrollment, or vocational goals

Ongoing encouragement to sustain employment or education

Medication & Wellness Support
Education and assistance with medication management

Promoting consistency and understanding of treatment plans

Encouragement of overall mental and physical wellness

Community Integration & Peer Support
Access to transportation for non-medical needs

Connection to community resources and social supports

Peer support to foster engagement and reduce isolation

Who Qualifies for the RISE Initiative:
Adults ages 18 and older enrolled in Kentucky Medicaid

Individuals with a qualifying diagnosis of serious mental illness (SMI), such as schizophrenia, bipolar disorder, or major depression

Those who would benefit from structured support to live independently in the community
